I will be retiring from Longshore Surgeries at the end of April 2022. I have thoroughly enjoyed my career at Longshore Surgeries which has spanned over 3 decades. We are fortunate to have dedicated and supportive staff who have been great to work with.  It is the intention of the practice to replace me when I leave, and I wish you all the very best for the future. Online Access to Medical Records

From 31st March 2016 the Practice will offer the facility for patients over the age of 16 to have on-line access to all coded information in their Medical Record. This includes medications, allergies, lab results, immunisations, problems and consultations. To apply for online access to your Medical Record, please read the patient leaflet and if you choose to proceed, please complete the application form and bring back to the surgery with the required forms of ID. (There is also a version of the patient leaflet with a proxy application form if this is appropriate).
